Amaç: Epileptik nöbet sonrası gelişen omurga kırıklarının görülme ve yerleşim yeri sıklığı sonuçlarımızı sunmak.
Yöntem: Bu çalışma Ocak 2010-Mayıs 2014 tarihleri arasında epileptik nöbet sonrası travmaya bağlı vertebra kırığı olan, üç merkezdeki hastaların (N=340) kayıtlarının retrospektif olarak incelenmesi ile yapıldı. Çalışmaya epileptik nöbet sonrası nörolojik ve radyolojik muayenelerine göre omurga kırığı tanısı olan hastalar dahil edildi.
Bulgular: Vertebra kırığı tespit edilen 40 hastanın 8’sinin servikal bölgede kırığı vardı: bunların da 2’sinde korpus kırığı, 6’inde hasta spinöz proseslerinde kırık tespit edildi. Torakal bölgede toplamda 17 hastanın 13’ünde korpus kırığı ve 4’ünde spinöz proseslerinde kırığı tespit edilirdi. Lomber bölgede ise 15 hastada kırık tespit edildi. Bunların 4’ünde korpus ve 11 ise spinöz proseslerinde kırık tespit edildi. Epileptik nöbet sonrası korpus yaralanması en sık torakalde ve proses yaralanması ise en sık lomber bölgede görüldü. Çıkan sonuçlar %95 güven aralığında idi, P=<005 olarak değerlendirildi.
Sonuç: Özellikle epileptik nöbet sonrası vertebra kırıklarında, korpus kırığı torakal bölgede, ve spinoz proseslerin ise lomber bölgede olduğunu ve epileptik hastalarda bunun göz önünde bulundurulmasının yararlı olacağını vurgulamak istedik.
Aim: In this study, we reported our results on the incidence and location of spinal vertebral fractures due to trauma following epileptic seizure.
Methods: This study is a retrospective analysis of the records of patients (N=340) in three clinics who had spinal vertebral fractures between January 2010 and May 2014. Of these, 40 patients who showed vertebral fracture indications due to trauma following epileptic seizure upon neurological and radiological examinations were included in the study. The rate and types of spinal vertebral fractures in these 40 patients was evaluated.
Results: Of 40 patients diagnosed with vertebral fractures, 8 had fractures in the cervical area, 2 had corpus fractures and 6 had spinous process fractures. Out of 17 patients with thoracic area fractures, 13 cases were corpus fractures and 4 cases were spinous process fractures. 15 patients had lumbar area fractures, 4 of which were corpus fractures and 11 process fractures. The highest incidence of spinal vertebral injury due to epileptic seizure trauma was observed in the thoracic area, whereas the highest incidence of process injury was observed in the lumbar area. A p-value of <0.05, with a 95% confidence interval, was considered to be significant.
Conclusions: We wish to emphasize that, especially in post-epileptic seizure spinal vertebral fractures, vertebral corpus fractures are observed in the thoracic area, whereas spinous processes are observed in the lumbar area; this should be taken into consideration in approaching such patients.
